I want to gently note something first: it sounds like some of this might be coming from guilt or your doctor's comment rather than something you actually want for yourself - that's worth being honest about, since goals that come from guilt tend to be harder to sustain than ones that feel like your own choice.
In the meantime, I'd start small regardless: a 10-minute walk at a specific point in your day you already have consistently, like right after lunch, rather than anything bigger - that gives it an easy anchor and doesn't depend on motivation being high that day.
